How to Use the Pet Sitting Forms

First, the pet sitting form has space to note our pet’s basic information.

  • their name
  • date of birth
  • age
  • breed
  • microchip number
  • current weight
  • whether they are male or female

There is also room to write down our information as the owner. We can leave our name(s), relevant phone numbers, and address.

The next section on the sheet is for documenting our vet’s contact information. There is space to write the phone number and address of our regular vet, along with info for an emergency vet in case the need should arise.

Tips for Using the Pet Sitter Printable

Since our pet’s information will generally stay the same over time, this printable could easily be filled out, laminated, and used over and over again.

We’ve had this thermal laminator for years and use it all the time to help make our printables reusable!

If you find you want to add more notes for a specific sitter, you can always use a dry erase marker to write extra information on the laminated sheet.

Keep the laminated sheet in a convenient location like a kitchen drawer or cabinet, hung on the fridge, or placed in your home’s command center. Then simply pull it out any time a sitter comes over or you’re taking your pet to the sitter’s.

Duties will vary by pet and could differ depending on the preferences of the owner and how long they will be away. Pet sitters are generally responsible for:

  • Feeding the pet and making sure they have water
  • Playing with the pet, making sure they get exercise, and walking them if necessary
  • Changing the litter box or other waste removal
  • Bathing, brushing, and grooming the pet
  • Making sure the pet is safe and healthy
  • Providing or arranging medical care for the pet if necessary
  • Maintaining routines that the owner has established for the pet
  • Showing the pet love, care, and affection
  • Communicating with the owner about the pet and sending photos if preferred/necessary
  1. Be sure to schedule a pet sitter well in advance of your trip, as they tend to fill up quickly. If you schedule early, you’ll be more likely to secure your preferred sitter for your pet.
  2. If the pet hasn’t met the sitter before, schedule a time for them to get to know each other when you can be present to let the pet become familiar and bond with a new person.
  3. Obtain and prepare any necessary food and snacks that the pet may need while you are away, especially if the pet requires a special diet.
  4. Secure any equipment, toys, crates, travel accessories, or other items that the pet sitter may need to use.
  5. Gather and organize any medications the pet may need while you’re gone. Be sure to leave clear instructions for administering the medications.
  6. Put away any items you don’t want the pet to be able to access while you’re away and block off access to areas of the home you don’t want them to navigate. You don’t want to come home to a huge mess!
